Choices

service and support to adult members of the behavioral health system

Choices

Choices Progam provides services and supports to adult members of the behavioral health system.  Members who are ready to become employed are referred by their CSP (Comprehensive Service Provider) Case Manager at La Frontera, Codac, Cope, Pantano or Providence.  Members attend orientation and assessment appointments and then develop their Individualized Plan for Employment with Choices staff.  Members are then enrolled in services that will assist them in meeting their employments goals ie: Computer Training, Internships, Food Fundamentals, and Job Search Skills classes.  All members are assigned to a Job Developer that will work in partnership with them to further increase their skills and obtain employment or other meaningful activities in the community.

How to qualify:
In order to get started a member needs current AHCCCS coverage and a completed referral packet from their CSP Case Manager at La Frontera Center, Codac Behavioral Health Services, Cope Community Services, Pantano Behavioral Health Services, Providence Service Corporation or Marana Health Center.  Members then attend orientation and assessment appointments and then, if the program is determined to be appropriate to meet the members needs, they develop their Individualized Plan for Employment with Choices staff. At that time they will be enrolled in the specific services (Work Adjustment and Internships, Food Fundamentals, Employability Skills Classes and Online Job Search/Computer Skills classes) that will assist them in meeting their employments goals.  All members are assigned to a Job Developer that will work in partnership with the member to further increase their employability and to obtain employment or other meaningful activities in the community.
